Family Practice Care

It is advisable that all Albertans have their own Family Physician that can help manage their health.  Our Family Physician can help manage incidental illnesses and chronic diseases, they act as gatekeepers who can work with you and a variety of specialists as required to keep you in good health.

We also provide walk-in services when needed. Patients can walk-in during our regular hours of operation.

We have a variety of qualified specialists in various fields of medicine. Our aim is to provide high quality and friendly service to all of our patients and clinics in our proximity.  We feel it is very beneficial to some of our patients with chronic conditions if we can secure specialists of the highest caliber to work closely with our Family Physicians.

Uninsured Services

 

We strive to make your experience the best it can be and for this reason we do offer services that are not covered under Alberta Healthcare.  Some of these uninsured services include:

  • Sick Notes
  • Pre-Employment Medicals
  • Driver’s Medicals
  • Insurance Medicals
  • Executive Health Examinations
  • Occupational Health

Immigration Medical Examinations:

Dr. Safdar Hussain, MBBS is Panel Physician Authorized by Citizenship and Immigration Canada (CIC) to perform Medical Exams needed to obtain Immigration/Visas/Permits. We are accepting appointments to do Medicals for the following:
-Work Permit: Including the removal of restrictions

  • Study Permit
  • Express Entry
  • Visitor Visa
  • Live in Care Giver

For all other applications please wait until you receive an IMM1017 form from CIC. Once you receive this form you will need to print it out and bring it with you for your appointment.

Diabetes Educator/Pharmacist

We have partnered with Primary Care Network to provide services that will help our patients address their conditions or concerns regarding their diabetes, medications or dosage changes. Our patients will work directly with a Registered Diabetes Educator/Pharmacist from PCN, who visits our clinic on a weekly basis. Please speak with one of our physicians to determine if working with a Registered Diabetes Educator/Pharmacist will help.

We welcome patients covered under WCB.

Freedom Sleep and Respiratory Care

Do you have questions about sleep apnea, sleep testing, or treatment?
Sleep apnea is a serious sleep disorder where breathing repeatedly stops and starts.  You may have sleep apnea if you suffer from:

  • Snoring
  • Daytime fatigue
  • Night-time choking
  • Irregular berating
  • Morning headaches

Please speak with one of our physicians if you are experience any of the symptoms listed above, they will refer you to Freedom Sleep and Respiratory Care Clinic.

Chronic Disease Management

If you are suffering from chronic diseases such as hypertension, diabetes, asthma, hyperlipidaemia, COPD, and obesity to speak with one of our physicians, they will refer you to a Chronic Disease Management nurse.

MOSAIC PCN After-hours Clinic and Services

This is a service that is available to our Clinics’ patients who need to see a Family Physician outside of clinic hours of Marlborough Medical Clinic

Referral through Health Link Alberta at 811 or through your Mosaic PCN physician. Location provided upon referral.

The Mosaic Primary Care Network (PCN) Access and Continuity of Care Extended Services (ACCESS) Clinic operates seven-days-a-week and provides after-hours care and same-day appointments for patients who need to be seen within four to 24 hours. Appropriate referrals include issues or concerns regularly seen in family physician offices.

To get a scheduled appointment, please call Health Link Alberta at 811. Let the nurse know you are a patient of a Mosaic PCN physician or mention the area of the city that you are in and ask if an appointment at the after-hours clinic is right for you.

Clinic Hours of Operation:
Monday – Friday, 5:30 – 9:30 p.m.
Saturday, Sunday, and all statutory holidays: 11 a.m. – 4 p.m.

The MOSAIC Clinic also offers Patients with number of Health classes and workshops and provides patients access to number of government programs including access to Mental Health Services and Social workers. Please follow the link provided to get further information
